We used xhtmlmydesign.com recently for couple of our projects. The code was structured and semantic. The css was very well organized, one of the best I have seen so far. They are not cheap but very reasonable compared to other services of comparable quality.

just in case anyone is foolish enough – like i was – then steer clear of rapidxhtml – they’re not on this list and probably good reason. i paid them $450 a week and a half ago, they’ve ignored my emails and ignored my request for a refund and not produced anything – not really what you need when using a service like these
